Internal Revenue Service SS4
This is a link to the IRS which will allow you to apply for your Federal ID number, known as your EIN. You may call 800-829-4933 7:00-10:00 your local time M-F to receive your number immediately. You will probably want to print this form out so you will have all the information the assistant will need. You will need Acrobat Reader 5.0 or greater.
EFTPS - Federal
This is the link to register for paying your Federal Income Taxes or your Payroll Taxes online. Click on Enrollment box right under EFTPS. Don't forget to print the confirmation. You will need Acrobat Reader 5.0 or greater.
Federal Tax Authorization Form
This is a link to the Internal Revenue Service form that needs to be filled out if you are having someone else do your Federal payroll taxes. It is fillinable but will have to be printed, signed and mailed. You will need Acrobat Reader 5.0 or greater.

Oregon New Hire Reporting Form
This is a link to the Department of Justice form you need to file when you hire a new employee in Oregon. You will need Acrobat Reader 5.0 or greater. If you have Enhanced Payroll with QuickBooks you already have this form on the state reports. If you don't have Enhanced Payroll ask us how you can get it at a reduced price.
Oregon Combined Employer Registration Form
This is a link to the Department of Employment form you need to file when are planning on having employees in Oregon. Fill in this form and fax it to the number on the 1st page. You will need Acrobat Reader 5.0 or greater.
EFT - Oregon
This is the link to register for paying your Oregon Payroll Taxes online. Go to page 7, fill out the form and mail it or fax it to 503-947-2016. Don't forget to send a voided check with it.You will need Acrobat Reader 5.0 or greater.
